Heart Guide Aotearoa Cardiac Rehabilitation Service

Our Service

The Heart Guide Aotearoa (HGA) is a home based Cardiac Rehabilitation management programme for all people with cardiovascular disease. The HGA proves an option to participate in a home based cardiac rehabilitation programme within your own environment. A specialist nurse will make four visits to your home or workplace and discuss cardiovascular risk factors with you to reduce your risks of having further cardiac events.

The HGA objectives are to:

  • Improve quality of life for people at risk of or with all cardiac conditions
  • Provide cardiac rehabilitation and therefore reduce hospital re-admission rates for patients with cardiac disease
  • To provide a programme that prefer individualised care at home (8-12 week programme)
  • Provides clinical expertise, support and education to the client and their whanau/family
  • Multi-disciplinary approach of health professionals throughout secondary and primary health care
  • Ensure a smooth transition from hospital to home
  • Work across Counties Manukau Primary Health Organisations

  • Observations of blood pressure, weight, pulse, blood sugar levels etc

  • Offering Kaumatua/Kuia and cultural support
